How the Panama 2-2-3 schedule works

The Panama schedule (often just called 2-2-3) staffs a 24/7 operation with four teams on 12-hour shifts in a repeating 14-day cycle: two shifts on, two days off, three on, two off, two on, three off. Its popularity in healthcare, dispatch, process plants, and utilities comes from two features workers like: you are never on for more than three days in a row, and every other weekend is a full three-day break.

The catch is the speed of the rotation. If your team alternates between day and night blocks, your sleep timing is asked to flip every few days, far faster than a human body clock can follow. Why short night blocks are a special problem

The circadian clock re-times itself by roughly one to two hours per day under good conditions. A two-night block is therefore over before your rhythm has moved more than a token amount, and any shift it did make becomes a cost you repay on the following day block. Workers who try to fully flip for each night pair end up permanently mid-flip, which is the most fatiguing place to live.

This is why the standard night-shift advice you may have read, sleep 7:00am to 2:30pm every day and hold it, is right for long night runs but wrong for 2-2-3. Short blocks need a different tool.

The anchor sleep strategy

That tool is anchor sleep. Classic chronobiology work by Minors and Waterhouse showed that if part of your sleep falls in the same clock window every 24 hours, your circadian rhythms stay largely synchronised to that window even when the rest of your sleep moves around. The anchor holds the clock steady; the moveable remainder absorbs the schedule.

In practice, on 2-2-3 with night pairs:

  • Off days and day-shift days: sleep a normal night in your usual window, for example 11:00pm to 7:00am. This is your anchor at full length.
  • Between night shifts: sleep the morning block, roughly 7:00am to noon, which still overlaps the tail end of your anchor window, then add a pre-shift nap later in the day.
  • Never schedule sleep that misses your anchor window entirely if you can avoid it. An afternoon-only sleep is the least restorative option and lets your clock drift.

The result is a clock that stays day-oriented all cycle, so your days off feel normal immediately, at the cost of running the night pairs on split sleep. For two, at most three nights, that trade is worth it.

Napping your way through the night pair

Split sleep only works if the naps are planned rather than accidental. Two matter most:

  • The pre-shift nap: 90 minutes in the late afternoon or early evening before a night shift, one full sleep cycle, timed to end an hour or more before you leave.
